We’re so excited to welcome you and your furry (or scaled) family to Walk with Claire. Before we get started, we need a few quick forms filled out to keep everyone safe, happy, and on the same page.

Here’s What’s Needed:
Please take a few minutes to complete the two forms below. They’re required before we can begin any services.

1. Liability Contract
Outlines our responsibilities and your pet’s safety — the legal must-do.

2. Veterinary Care Release
This gives us permission to seek medical care in the rare event of an emergency.
